New
Foundations’ vision is to promote the development of
housing and supportive services in order to assist formerly
homeless families in the transition from shelters to permanent
housing. Our desire is to make meaningful and lasting changes
in our families’ lives, empowering them with the tools
that will enable them to build their lives on a new foundation
and move toward self-sufficiency.
In collaboration with the
U.S. Department of Housing and Urban Development, New Foundations
assists homeless, disabled single heads of household with
children by providing scattered site permanent supportive
housing and case management services.
To be eligible for the New Foundations program
a family must:
1)
Be a single headed household (man or women) 18 years
of age or older with one or more children under the age of
18 years old currently living with you. (If children are not
living with you, you must have a court order stipulating reunification
upon securing housing.)
2) Have at least one member of the household
who has been diagnosed with a permanent disability (physical,
mental or D/A dependency).
3)
Have verification of homelessness as defined by the
federal government.
Referrals
to the New Foundation Supportive Housing Program can come
from emergency or group shelters domestic violence shelters,
safe homes, mental health, drug or alcohol facilities, or
local Social Service agencies serving the homeless. Self-referrals
are not accepted.
